Application Checklist

Please make sure that you have done the following as you make application to the PA Program:

ð I am applying online between the dates of April 15^th and December 1^st..

ð I have contacted the SIUC PA Program Academic Advisor at (618) 453-5527, paadvisement-l@listserv.siu.edu or http://mccoy.lib.siu.edu/~paprogram for any updates.

ð I have the original GRE, MAT or MCAT Score Sheet.

ð I have my official transcripts showing coursework towards and/or a posted Bachelors Degree.

ð I have the Essay as described in the CASPA application packet.

ð I have at least three (3) completed SIUC MSPA reference forms* in envelopes that are sealed and signed across the back flap. (*forms are provided on Program website)

ð I have a resume or curriculum vitae.

ð I have the $50.00 application fee.

ð I have gone online to www.siu.edu/gradschl and completed the generic online information form.

ð I have contacted the SIUC PA Program Academic Advisor regarding any questions that I had on the Program or my CASPA application packet.

ð I mailed any supplemental materials so that they would be postmarked on or before December 1^st.

Application materials will not be accepted after the December 1^st deadline. Please check off each of the above items as they are completed.
